- Summary: the PCs explore the deepest, most dangerous reaches of the Underdark where ancient evils stir and the boundary between worlds grows thin

2. THE SURFACE WORLD (1-2 pages)
- The nearest surface settlement
- 4-5 locations and NPCs
- The surface world's connection to the Underdark

3. THE UPPER UNDERDARK (2-3 pages)
- 3-4 areas in the upper levels
- Various Underdark races and their territories
- The path downward

4. THE DEEP UNDERDARK (2-3 pages)
- 3-4 areas of the deep levels
- Ancient ruins, powerful entities, impossible geography
- Signs of the ancient evil's influence

5. THE ABYSSAL THRESHOLD (2-3 pages)
- 3-5 areas near the boundary between worlds
- Planar incursions and reality-warping zones
- The ancient evil's gateway or altar
- Environmental challenges and planar hazards

6. THE ANCIENT EVIL'S DOMAIN (1-2 pages)
- The entity's domain or realm
- The entity's appearance and personality
- Their plan and what happens if they succeed
- Boss encounter with planar abilities

7. STAT BLOCKS (1-2 pages)
- 20+ stat blocks for Underdark creatures, ancient entities, planar beings

8. DM GUIDANCE (1-2 pages)
- Running a deep Underdark adventure
- Sequel hooks
